In-home Support for Older Adults

Our in-home support services are designed for older adults who want to continue living at home, with the right level of help to support everyday life. We focus on the small, familiar moments that make home feel comfortable and predictable, from daily routines to moving through the week with confidence.

Whether you need a hand with personal care, help keeping the house in order, support getting to appointments, or time for carers to rest, we work with you and your family to shape support that fits naturally around your routines.

Services you can access

Support with daily personal care, health needs and meals so you can stay as independent as possible at home.

  • Personal care: help with showering, grooming, dressing and daily routines.
  • Nursing: clinical assessments, health monitoring and ongoing nursing support.
  • Allied health & therapy: physiotherapy, occupational therapy and other therapies, coordinated with your care team.
  • Meal preparation & diet: support with shopping, cooking and planning meals that suit your preferences.
  • Specialised support: more complex or tailored supports based on your health needs.

Practical help to keep your home safe, tidy and comfortable, plus extra support when you or your carer need a break.

  • Domestic assistance & house cleaning: regular support with cleaning and household tasks.
  • Home maintenance: light maintenance to keep your home safe and functional.
  • Goods, equipment & assistive technology: equipment that can make moving around or daily tasks easier.
  • Respite: short-term support so carers can rest and recharge.
  • Help to restore independence: support after illness, injury or a change in health.
  • Transition care: extra help after leaving hospital while you recover at home.

Support to stay socially connected and get to the places that matter to you.

  • Social support: company at home or support to get out to social activities, appointments and community events.
  • Transport: getting to and from appointments, shopping, social outings and other important activities.

How in-home support works

We start by understanding what a good week looks like for you at home. This includes your routines, priorities, and the kinds of support that would make day-to-day life easier. From there, we work with you to decide which services are useful and how often they should happen.

If you receive a Home Care Package, we partner with an approved provider who manages your funding. Together, we plan how those funds are used so support aligns with your priorities and remains flexible as circumstances change.

You will always know who is coming into your home, what they will be supporting with, and how to adjust support if your needs shift over time.
